ST JOHN'S EXCURSIONS & CAMPS

The below form is to be completed for each student attending an overnight camp OR day excursion where longer travel and catering is required.

Within this form we ask your child's information regarding;
- Administering of Medication - Dietary Requirements - Travel Instructions - Other Student Information
Once you've completed this form, please return to your event within Parent Lounge to 'accept' your child's attendance on the excursion.
NOTE: If your child is not attending the specific camp or excursion, you are not required to complete this form.
