Medication Request Form

Should your child require a regular or one-off administering of medication either at School or on excursion, St John's Lutheran School requires parental/care giver consent for us to administer this to your child.

PLEASE NOTE: All medication MUST be handed to the School, in it's original packaging, with a pharmacy label including your child's name and instructions from the medical practitioner.
